Punctured Lavage Solution Bag.
Porous Lavage Tubing Assembly.
Cracked Handpiece Adapter
Cracked Steri-Mate Handpiece.
Incorrect tubing size.
O-ring worn or missing on Cavitron
Insert.
O-ring worn or missing on Cavitron
Handpiece Cable.
O-ring worn or missing on handpiece
adapter.
Insert defective.
Steri-Mate Handpiece defective.
Steri-Mate Handpiece Adapter
defective.
CORRECTIVE ACTION
Replace lavage solution bag.
Replace tubing assembly.
Replace the handpiece adapter.
Replace the Steri-Mate® Handpiece.
Check the tubing for snug fit on the adapter
barb. If tubing is loose, replace it with the
correct tubing (.110 inch / 2.794 mm I.D.).
Replace the O-ring. Green (12-pack)
