5.1.1 Inserting tomas
-pins EP

is skeletally anchored in the palate by 2 tomas
-pins  EP. The
®
®
choice of pin length depends on the bone supply and the gingival
thickness. Most cases can be treated with 8 mm tomas
-pins  EP
®
(REF  302-208-00). The tomas
-pin  EP is available in two more lengths:
®
10 mm (REF 302-210-00) and 6 mm (REF 302-206-00).
The bone supply can be measured using X-rays. The middle line and the
third palatine ruga can help to determine the correct insertion point.
Trace an imaginary transversal line between the distal contact points
of the upper cuspids. If the cuspids are nonexistent or malpositioned,
use the third palatine ruga as orientation. The tomas
-pins EP should be
®
inserted 3 to 6 mm posterior to the incisive foramen. Position the two
tomas
-pins EP 3 to 4 mm left and right of the middle line, as parallel as
®
possible and in the same anterior-posterior plane. The pin heads should
sit at the same height. Insert the two tomas
-pins EP as perpendicular as
®
possible to the plane of the anterior palate. Hold the insertion instrument
at an inclined angle to the occlusal plane during insertion. Align the tips
of the tomas
-pin  EP with the posterior region of the root tips of the
®
upper incisors (Fig. 2).
Base the choice of pin length and insertion angle on the bone supply
and the space needed for a later retraction of the upper incisors. We
recommend the longer version of the tomas
-driver (REF 302-004-60)
®
