Reporting damage
To report damage:
1. Note any damage on the bill of lading and take pictures.
2. Keep shipping cartons, pallets, and other shipping materials to prove any damage
to the freight carrier. If possible, save an undamaged carton and pallet in the event
a recliner must be returned to the factory.
3. Contact Stryker Customer Service or Technical Support at: 1-800-327-0770
immediately to report the damage. All claims must be filed by the consignee and
reported within 45 days of receipt of shipment.
Return authorization
To return:
Stryker customer service department
return authorization number by fax or email.
All returns must be sent prepaid by the sender, except for provable shipping
damage.
Any recliner put into use will not be returnable due to the subject of infection
control.
Storage
To store:
1. Cover recliner and store in a dry area.
2. Do not place objects on top of the recliner that may damage the upholstery.
Before recliners are put into service, all personnel should review this manual. This manual should
remain with the product even if the product is sold, as this is a permanent part of the recliner.
WARNING: Improper methods of operating the recliner can damage the recliner and void the
warranty. Only operate the product as described in this manual. Modification of the product or any
components of the product is prohibited and can cause unpredictable operation resulting in injury.
Modifying this product also voids the warranty. If you cannot locate your in-service packet, contact
