Blom-Singer ® Dilation/Sizing System
sizing chart). Position the tapered tip of the dilator in the
puncture and gently insert the device completely to the neck
strap (diagrams 1a, 3a, 5, 6). Avoid touching the portion of the
dilator that will enter the puncture. Secure the dilator near the
edge of the stoma with adhesive tape across the neck strap to
prevent the dilator from slipping out. Note: if the puncture angles
straight back or slightly downward, insert the dilator such that its
curvature is also directed downward. If the puncture angles in
an upward plane, insert the dilator such that its curvature is also
directed in an upward direction. Remove the dilator if gagging
or discomfort is experienced; users should consult with their
physician or speech pathologist.
The dilator should be left in place for 5 minutes, or longer if
necessary, in order to sufficiently dilate the TE puncture and
allow for easy insertion of the voice prosthesis (Clinician: or
Voice Prosthesis Sizer). Remove the tape from the neck strap,
grasp the dilator securely, and carefully withdraw it from the
puncture. Immediately insert a voice prosthesis (Clinician: or
Voice Prosthesis Sizer).
Measurement of the TE Puncture Tract (Clinician)
Note: Always measure the TE puncture tract to confirm correct
voice prosthesis length before inserting a voice prosthesis.
Dilator-Sizer
The Dilator-Sizer, of the appropriate diameter (see sizing chart), is
inserted gently by the Clinician completely into the TE puncture
(diagram 6). Note: if the puncture angles straight back or slightly
downward, insert the dilator such that its curvature is also
directed downward. If the puncture angles in an upward plane,
insert the dilator such that its curvature is also directed in an
upward direction. Remove the dilator if the patient experiences
gagging or discomfort.
Carefully withdraw the Dilator-Sizer until very gentle resistance
is detected indicating that the esophageal wings (diagram 4)
are seated against the anterior wall of the esophagus (diagram
10). Note: Pulling too firmly on the Dilator-Sizer may squeeze or
compress the TE tract resulting in an erroneous measurement
that indicates it is shorter than it actually is.
