TWO YEAR LIMITED WARRANTY
CongratulationsonyourpurchaseofaVeridianHealthcarePulseOxFitPremiumPulseOximeter.Your
PulseOxFitPremiumPulseOximeter.iscoveredbythefollowinglimitedwarrantycommencingupon
thedateofpurchaseforaperiodoftwoyears,andsubjecttothefollowingtermsandconditions:
VeridianHealthcarewarrantsthatitsPulseOxFitPremiumPulseOximeterwillbefreefromdefectsin
materialsandworkmanshipundernormalconsumerusageforaslongastheoriginalpurchaserowns
theproduct.
Periodicmaintenance,repairandreplacementofpartsduetonormalwearandtearareexcludedfrom
coverage. Defectsordamagethatresultfrom:(a)improperoperation,storage,misuseorabuse,
accidentorneglect,suchasphysicaldamage(cracks,scratches,etc.)tothesurfaceoftheproduct
resultingfrommisuse;(b)contactwithliquid,water,rain,extremehumidityorheavyperspiration,
sand,dirtorthelike,extremeheat,orfood;(c)useofthePulseOxFitPremiumPulseOximeterfor
commercialpurposesorsubjectingthePulseOxFitPremiumPulseOximetertoabnormalusageor
conditions;or(d)otheractswhicharenotthefaultofVeridian,areexcludedfromcoverage.This
warrantydoesnotcoverbatteriesorotherpowersourcesthatmaybeprovidedwith,orusedwiththe
PulseOxFitPremiumPulseOximeter.
IfthePulseOxFitPremiumPulseOximeterfailstoconformtothislimitedwarranty,returnthePulseOx
FitPremiumPulseOximeterpostageprepaidalongwith$5.00forreturnshipping,handlingand
insuranceto:VeridianHealthcare,Attn:RepairDepartment,1175LakesideDrive,Gurnee,IL,60031.
Whenreturningaproduct,pleasealsoinclude:(i)acopyofyourreceipt,billofsaleorothercomparable
proofofpurchase;(ii)awrittendescriptionoftheproblem;and(iii) yourname,addressandtelephone
number. Carefullypackagetheproducttoavoidanydamagethatmayoccurwhileintransit;shipping
insurancewithreturnedreceiptisrecommended.Atouroption,VeridianHealthcarewillrepairor
replacetheunitfoundtobedefectiveinmaterialsorworkmanshipundernormalconsumerusage.
Thepurchaserwillbenotifiedofanyadditionalrepairsrequiredpriortocompletingtherepair,andwill
beresponsibleforpartscharges,ifany,andrepairchargesnotcoveredbythislimitedwarranty.
